Care Coordinator

Job Description

 General:
The Care Coordinator is responsible for assessing the needs and coordinating the treatment services for program participants. Case management experience required. CFT experience a plus. Spanish speakers are encouraged to apply! Come join our exciting and creative team!

Specific Responsibilities:

* Screens and evaluates clients for whom treatment is being requested.
* Coordinates intake and referral procedures with agency staff and other service providers.
* Develops and monitors treatment plans with clients and participating agencies.
* Assists clients in securing a variety of social services.
* Maintains client records in accordance with agency and funding source requirements.
* Provides complete documentation and reporting of all client service in accordance with agency and funding source requirements.
* Meets with treatment providers to review utilization and monitor client progress toward individual treatment goals.
* Attends and participates in team meetings and clinical staffing.
* Performs crisis intervention and solution-focused interventions; acts as liaison with providers of high-acuity services.
* Participates in ongoing program planning and development.
* Performs other related activities in accordance with agency growth and changes.



Skills/Requirements

 Education & Experience:

* Bachelor's Degree from an accredited institution with a major in psychology, counseling, or related behavioral health field and two years of related experience.
* Knowledge of DSM-IV.
* Knowledge of community resources.

General Skills and Requirements:

* Strong computer skills including ability to effectively utilize Microsoft Office programs such as Excel and Word
* Ability to interact effectively in a team setting with other behavioral health professionals and with other service providers.
* Ability to communicate effectively, both orally and in writing.
* Knowledge of various ethnic and cultural groups.
* Valid Arizona driver license and willingness to use personal vehicle in the course of employment.
* Willingness and ability to work in other organizations' facilities and in clients' homes.
